NEW GUIDELINES FOR REPATRIATION OF CORPSE TO NIGERIA

SUBMISSION OF A LETTER OF REQUEST FOR A WAIVER FOR THE REPATRIATION OF CORPSE ADDRESSED TO THE HONOURABLE MINISTER OF HEALTH AND SUBMITTED TO PORT HEALTH
SERVICES (PLEASE NOTE THAT THE APPLICATION SHOULD BE WRITTEN BY THE RELATIVES OF THE DECEASED OR OTHER AUTHORIZED RE PRE SENTATIVES).

8. THE REQUEST FOR THE WAIVER SHOULD PROVIDE INFORMATION ON THE CAUSE OF DEATH AND BE ACCOMPANIED WITH THE FOLLOWING DOCUMENTS:

  1. DATA PAGE OF THE DECEASED INTERNATIONAL PASSPORT
  2. DEATH CERTIFICATE
  3. EMBALMENT CERTIFICATE
  4. EVIDENCE OF PAYMENT OF N5,161.25 (FIVE THOUSAND, ONE HUNDRED AND SIXTY-ONE NAIRA, TWENTY-FIVE KOBO) TO THE PORT HEALTH SERVICES TSA ACCOUNT (052102900100 – TENDER)

BESIDES THE ABOVE, KINDLY NOTE THAT THE NCDC GUIDELINES FOR THE SAFE TRANSPORTATION OF REMAINS OF PERSONS SUSPECTED OR CONFIRMED TO HAVE DIED OF COVID-19 CAN BE ACCESSED ON THIS WEBSITE: www.ncdc.gov.ng

NOTE: Allow 7 working days for processing time. 

REQUIREMENTS FOR LEGALIZATION OF PRODUCTS

1. Application (on your company’s letter head) stating your request from the Embassy and list of the products to be exported.

2. Two sets of the documents to be legalized, i.e. original and a copy for our record.

3.  155, 000 fcfa per product payable to the Embassy’s Account at First Bank.

NOTE: Allow 7 working days for processing time. Also, drop off and collection of documents from Monday through Thursday between 11:00am to 12:30pm.

REQUIREMENTS FOR AUTHENTICATION OF NIGERIAN PASSPORT

  1. Print duly filled authentication and registration form, available online.
  2. Submit printed application and registration  form along with  photocopy of passport or police report in case of loss of passport 
  3. A copy of Nigerian passport data page as well as the original passport (to be sighted and returned to owner)..
  4. Fee of 5000fcfa administrative charges  payable to the Embassy’s Account at First Bank.  

NOTE: Allow 7 working days for processing time.

REQUIREMENTS FOR ISSUANCE OF CONSULAR CARD 

1. Print duly filled Consular Card application and registration form available online and payment of 10, 000 FCFA to the Embassy’s Account at First Bank.  

2. Submit printed application and registration form, photocopy of passport or police report in case of loss of passport 

3. In case of loss passport, applicant should produce proof of identity 

4. A passport photograph

NOTEAllow 7 working days for processing time. Also, drop off and collection of documents on Mondays and Wednesdays between 9am to 1pm.

REQUIREMENTS FOR ISSUANCE OF POLICE CHARACTER REPORT  

  1. Print duly filled   Police Report and registration form application available online and payment of 5, 000 FCFA to the Embassy’s Account at First Bank. 
  2. Submit printed application and registration  form along with proof of payment, photocopy of passport or police report in case of loss of passport 
  3. In case of loss of passport, applicant should produce proof of identity 
  4. A passport photograph

NOTEAllow 7 working days for processing time. Also, drop off and collection of documents from Monday through Thursday between 9am to 1pm.

REQUIREMENTS FOR LEGALIZATION OF DOCUMENTS 

  1. Print duly filled Legalization form and registration application available online and payment of 5, 000 FCFA to the Embassy’s Account at First Bank. 
  2. Submit printed application and registration  form along with photocopy of passport or police report in case of loss of passport 
  3. Documents issued in Nigeria must be authenticated by appropriate authorities in Abuja
  4. In case of loss passport, applicant should provide proof of identity 
  5. A passport photograph

NOTEAllow 7 working days for processing time. Also, drop off and collection of documents from Monday through Thursday between 9am to 1pm.
